It's been long expected that the South Australian would make the step up at the main game next year, and was likely to do so with MSR, the team that took him to this year's second-tier title.
It's now been confirmed that the deal will go ahead, MSR having secured the use of Jason Bright's Racing Entitlements Contract, which was used as the fourth entry at Prodrive this year.
MSR will run a single ex-DJR Team Penske FG X Falcon for Hazelwood, the car having been purchased by well-known racing supporter James Rosenberg, while funding will be underpinned by IT entrepreneur Jason Gomersall and his iseek Communications business.
"After such an incredible 2017 season, there is no better time to make this transition and start an exciting new chapter of my career," said Hazelwood.
"I am more than ready for this and I'm so fortunate that we have been able to achieve this deal.
"I am so grateful to have such incredible people in my corner like Matt, Jason and James and cannot wait to see what we will achieve in 2018."
The Hazelwood/MSR deal brings the Stone name back to Supercars, Matt Stone the son of Jim, one half of the famous Stone Brothers that dominated the sport with Marcos Ambrose and Russell Ingall in the early 2000s.
"The success we achieved with Todd and the team in 2017 demonstrates we are more than ready to make this exciting transition," said Matt Stone.
"The Matt Stone Racing team have tasted success in each category we have competed in and it is very special to see our team on the main series grid next year.
"We are prepared and have the key ingredients to be successful when we arrive in Adelaide in 2018."
The deal is part of the silly season shake-up that will ultimately see Richie Stanaway confirmed as the fourth Prodrive driver for next season in an ex-LD Motorsport REC.
